Alameda Hospital at South Shore
Rehabilitation and Wellness Center
Alameda Hospital at South Shore provides specialized, extended skilled nursing and rehabilitative care in a safe, welcoming environment. With an emphasis on maintaining quality of life and the dignity of our residents, our small size (only 26 beds) allows us to provide warm, individualized care in a home-like setting. Our compassionate and professional staff provides 24-hour nursing care, including assistance with daily living tasks such as dressing and personal care. Community activities include everything from art classes to pet visits. Alameda Hospital at South Shore also provides hospice and respite care, so we are able to meet the needs of every senior we care for.

Subacute Care Program
Specialized Inpatient Services
Alameda Hospital's Subacute Care Program provides care for patients who require specialized services more intensive than long-term or skilled nursing care, but less so than acute hospital care. Here, patients benefit from the collaboration of an interdisciplinary team. Our Medical Director, Registered Nurse, Respiratory Therapist, Rehabilitation Specialists (PT, OT, and SLP), Social Worker, Dietitian, and Activity Director work together to develop an individualized plan of care for a wide variety of needs: ventilator dependency, tracheostomies with mist/oxygen, tube feeding, IV therapy, IV med administration, wound care, inhalation therapies, and physical, occupational and speech therapies. Family members are encouraged to play a vital role in the plan of their loved one's care and receive frequent progress reports.
